Transcripts and Marks Sheets

Applicants should complete the SOPHAS Official Transcript Request Form (found at www.sophas.org) and send it to all post-secondary college and universities attended. This Transcript Request Form must accompany all transcripts. All post-secondary transcripts must be sent from the registrar or designated school records official directly to SOPHAS. Transcripts sent from applicants will not be accepted by SOPHAS. Photocopies of official transcripts or unofficial records will not be accepted.

We recommend that you request two sets of official transcripts/marks sheets at the time of application. Please submit one set to SOPHAS, which will be used for the application review. Please note that SOPHAS retains the official set of documents that you send SOPHAS. Please keep the other set unopened. If you are admitted, you may submit this unopened set in order to satisfy official transcript contingencies.

If you are currently in a degree program, you must submit a current official transcript. If you are accepted for admission, a final official transcript showing final course grades, the degree awarded, and the date of degree conferral must be submitted prior to enrolling.


Applicants who have attended post-secondary institutions in which English is not the official language of instruction must submit official transcripts or marks sheets in the original language as well as an official English translation.

Applicants with Non-US Transcripts/Marks Sheets/Degree Conferral Documents

Applicants who will be submitting documentation from post-secondary institutions outside of the United States may choose to have their documents evaluated by an external credential evaluation agency. While we recommend that students use a credential evaluation agency, we do not require it.

Harvard School of Public Health will accept either:

-original/official documentation with English translation sent directly from the post-secondary institution to SOPHAS

or

-evaluations from the credential evaluation agencies posted at http://www.sophas.org/documents/SOPHAS_Intl.pdf
