Dear Current Student,
The following information will guide you through your change in degree or department. The first section is for current students requesting a degree/department change. The second section is for current Environmental Health students requesting a change in concentration within the department.
Regardless of the changes you wish to make, you must have a conversation with your current advisor prior to requesting a change. In addition, you must also have conversations with both your current and prospective departments' academic administrators.

Your request for change will not be submitted for review until the following forms have been completed and received by the Admissions Office:
Current Students Requesting to Change their Degree or Department
1. Application Short Form (two pages)

For change in degree: Please ensure you meet any prerequisite degree requirements for your proposed change. Please also make sure you have a conversation with your current advisor prior to requesting a change. In addition, you must also have conversations with both your current and prospective departments.
2. Two Official Letters of Recommendation-one from the current department, and one from the new department. Emailed recommendations must be submitted from an HSPH email address to klatendr@hsph.harvard.edu.
For change in degree within the same department: You need to only submit one letter of recommendation
3. Updated Statement of Purpose: Typed and double spaced, approximately 500 words in length and should describe the following:
- Your academic and/or professional preparation for a career in public health.
- Your reasons for wanting to enroll in the degree program or department to which you are applying.
- Your career plans upon completion of the program.
4. Updated CV/Resume

For any changes, all decisions must be reviewed and approved by the Committee on Admissions and Degrees. You will then be notified of the decision by the Admissions Office.
