For students who matriculated prior to September 2003, the first transcript is issued at no charge; for subsequent requests, there is a processing fee of $2.00 for each transcript. Students matriculating after September 2003 have been charged a $10.00 Academic Records fee, which covers the cost of transcripts. All transcript requests must be made in writing by either submitting a Request for Transcript Form to the HSPH Registrar's Office or writing to the HSPH Registrar's Office. To request a copy of your official transcript, please include:
- your name
- degree program
- dates of attendance
- date of birth
- signature
- check for $2 per transcript (not required for first transcript requested) for students who matriculated prior to September 2003.

Checks should be made payable to the Harvard School of Public Health.
Please allow five to seven business days to process requests for current students and recent attendees. Allow seven business days for attendees prior to 1990.
Transcripts issued to students with outstanding financial obligations to the University will be marked "student's financial obligations to the University have not been met." These transcripts will not be sent to third parties.
In accordance with federal law, records cannot be released without the consent of the student. Official copies of transcripts will not be transmitted by fax or e-mail.
