Department of Epidemiology Forms
EPI Department Required Courses: Students wishing to request a waiver for departmental required courses (not HSPH required courses) must submit the EPI Requirement Waiver Form (obtained from the Epidemiology department). This form should be submitted to the Academic Services Coordinator along with a copy of the syllabus and a transcript from the institution where the course was taken. The student and their academic advisor will be notified of the decision on the waiver and a copy will be placed in the student’s academic file.
