New iSite for Foreign Nationals

Reprints of the 1042-S Tax Forms and More….

The NRA (Nonresident Alien) Tax group has created an iSite for foreign nationals. The site is in it’s first phase and is intended to provide foreign nationals with a central place to request reprints of a prior year 1042-S issued by Harvard and more (see below).

HSPH Spring Challenge: Bicycle Safety: Boston and the World Apr 11-19

Join us for a rewarding experience as a Team Advisor or Competition Judge – Limited Time Commitment

The HSPH Center for Public Health Leadership announces the second annual HSPH Spring Challenge public policy case competition. You are invited to join in this school-wide, leadership and policy development exercise for students. We need staff volunteers to serve as Student Team Advisers or Competition Judges. The time commitment is limited. The competition will happen from April 11-19, 2013, and we only need two to four hours of your time during those eight days.

Our 2013 Spring Challenge topic is bicycle safety. From 29 suggestions across the School, HSPH students voted for this topic as a rich and compelling one. No expertise in bicycle safety is needed to serve as an advisor or judge, only a passion for public health innovation and student learning.

Ashok Alexander led the Bill & Melinda Gates Foundation’s India office from its inception in 2003 until 2012, where he created Avahan, the Foundation’s flagship India AIDS prevention program. Avahan, which became probably the largest private prevention program ever undertaken in HIV/AIDS, scaled up Avahan across six states in India within three years. Avahan worked with groups most at risk to HIV and rolled out community led prevention in more than 500 towns in the six states, and across the national highway system, showing how business thinking could be used to scale up public health delivery. Ashok’s team took the business model behind Avahan to new public health prevention areas such as maternal and child health and infectious diseases. Over a decade, Ashok led the growth and expansion of the Gates Foundation’s first country office, with a grant portfolio of over $1 billion spanning health, sanitation and agriculture.
Prior to joining the Foundation, Ashok worked in the private sector for 24 years in Hong Kong, the United States and India, including serving as Director of McKinsey and Company and head of the consulting firm’s New Delhi office. Ashok is a graduate of St. Stephens College in New Delhi, has a post-graduate degree from the Delhi School of Economics, and an MBA from the Indian Institute of Management in Ahmedabad. He is a founding board member of the Public Health Foundation of India, serves on the board of CARE India, and is a founding trustee of the America-India Foundation. Currently, Ashok is engaged in developing Antara, a non-profit dedicated to health delivery at scale. For Spring 2013 he is a Menschel Senior Leadership Fellow at the Harvard School of Public Health.

Kathleen Sebelius is the 21st Secretary of the Department of Health and Human Services (HHS). Before her Cabinet appointment in April, 2009, she served as Governor of Kansas beginning in 2003, where she was named one of America’s Top Five Governors by Time Magazine. From 1995 to 2003, she served as Kansas Insurance Commissioner. She was a member of the Kansas House of Representatives from 1987 to 1995. Forbes has named Secretary Sebelius one of the 100 most powerful women in the world.

New I-9 Form Effective Immediately

All HSPH I-9 Creators Should Begin Using New Form

The I-9 Form has been revised by USCIS. Effective immediately, you should begin to use the new form which can be downloaded here. It has also been updated on the HR iSite, on the OFA website, and in the Wasabi system.

For I-9 forms in transition (paperwork in process), Central Payroll will continue to accept the older I-9 form (Rev. 08/07/09) up until May 7, 2013. After that date, any forms sent in the older format (Rev. 08/07/09) will be returned back to us.

This Week: ESTR Monthly Lab Training (Hands on tutorial on how to submit through the IRB’s new electronic system)

Intermediate Session:
This session will cover specific information about entering a modification or continuing review, along with some tips and tricks for using ESTR.

These training session will cover a brief overview of ESTR & the submission process from start to finish, highlighting some of our Frequently Asked Questions:

• Uploading Study Documents (When, where, and which supporting documents need to be uploaded, and clarification of update versus delete)

• Funding Source (Review process for searching for funding and adding new funding including adding supporting grant documentation, when appropriate)

• NHSR and Exemptions (What to attach and how to manage the revised clarification process through ESTR modification)

• Submitting to IRB (Who must/can submit (when coordinators hit submit where does it go), submission process during IRB clarification Request – when are changes actually submitted to IRB)

• Modifications (Review process for Modifications, Mod/CR, and Personnel Modification)

o Adding Study Personnel (understanding process for when Personnel form is required vs. when personnel should be adding through ESTR).

These will be targeted at all applicants (Faculty, Students, and Staff) who will work in the ESTR system.

A Toastmasters meeting is a learn-by-doing workshop in which participants hone their speaking and leadership skills in a no-pressure, fun atmosphere.
