New Computer Setup Request Form

This form is used to request the connection of a new computer to the HSPH network. It must be filled out completely and submitted to the Helpdesk. Also please note that new hardware should not be taken out of the box or started prior to the technician’s arrival. Upon receipt of your completed for the Helpdesk will contact you to schedule the setup.
