Patient safety expert calls on health care institutions to better manage adverse events

In the seventeen years since Boston Globe health columnist Betsy Lehman died due to a medication error at Boston’s Dana-Farber Cancer Institute, the institution has transformed its culture, according to a November 15, 2011, Boston.com blog post by HSPH’s Lucian Leape. A health policy analyst whose research has focused on patient safety and quality of care, Leape calls for the rest of the health care industry to follow Dana-Farber’s example and adopt more transparent practices for dealing with adverse events.
