Initial Protocol Submission Forms	Instructions
Initial Application for Human Research Form	This form is no longer in use.  Instead, for all studies, log into ESTR: irb.harvard.edu and select “Create New Study.”
Research Protocol	Submit this document for any new human research activities including exemptions via ESTR: irb.harvard.edu (Note that this form is not required for Not Human Subjects Research Requests).  The Research Protocol will also need to be updated when requesting modifications where applicable.
Secondary Use of Research Samples/Data Application	This form is no longer in use.  Instead, for all studies, log into ESTR: irb.harvard.edu and select “Create New Study.” (Note that a Research Protocol is now required for all Human Subjects Submissions.)
Exemption Request	This form is no longer in use.  Instead, for all studies, log into ESTR: irb.harvard.edu and select “Create New Study.” (Note that a Research Protocol and “Study Team Members” list are now required for all Human Subjects Submissions, including Exemptions.)
Not Human Subjects Research Determination Request	Investigators are responsible for not conducting Human Research without prior IRB review and approval. If investigators have questions about whether an activity is Human Research, contact the IRB office.  If an investigator wishes to obtain a written determination, log into ESTR: irb.harvard.edu and select “Create New Study.” (Be prepared to upload the Not Human Subjects Request Form in lieu of a Research Protocol.)
