Photo IDs

Harvard University has a University wide photo I.D. card system. The identification cards have a magnetic strip on the back which enables people to use their cards for meal plans, access to electronic locks and to check out library books. This card is necessary in order to access any Harvard University facility.

Each department administrator has I.D. request forms, which should be filled out by all new employees. In order to complete the form you must include your Harvard I.D. number which the payroll office will issue. Non-payroll faculties need to complete forms through the Office of Academic Affairs in order to obtain a University I.D.

Non-payroll consultants or vendors who require frequent access to the school will be issued a generic I.D. These identification cards expire annually. In order to process generic I.D. requests please submit the forms to Operations.

The HSPH I.D. Office is located in SPH-3, room 119. This is where you have your photograph taken for your I.D. The office is Open Monday through Friday, 8:00 a.m. – 4:00 p.m.

Please treat your I.D. as you would your credit cards or other valuable cards. If lost, promptly file a “Lost or Damaged” I.D. card form. These can be picked up at the ID office or filled out online at www.huid.harvard.edu under forms and brought to the ID office.
