Listed below are forms that are available at the HSPH Registrar's Office. Certain forms are available as printable PDFs. Forms that doctoral students are required to submit for approval by the CAD throughout their tenure at HSPH, as well as forms that may be used in specific situations, are located on the Doctoral Student Resource page.

Affiliate/Alumnus Registration Form
Used by Harvard affiliates and alumni to register for HSPH course(s).

HSPH Tap Registration Form
Used to participate in TAP Benefits at HSPH.

Cross-Registration Late Add/Drop/Change Form
Used to add/drop/change a cross-registered course by students cross-registering into HSPH.

Request for Certification Form
Used when requesting certification of HSPH enrollment, tuition and fees, or graduation.

Leave of Absence Petition
Used when requesting a leave of absence from HSPH.

Application for Degree
Used to apply for graduation. Student must provide name as it should appear on the diploma, degree program and department, previous degree(s), and the name(s) of the academic institutions which conferred the degree(s).

Diploma Dispersal Form
Submitted by graduating students to indicate whether or not they will be attending the HSPH Commencement Ceremony.

Request for Replacement Diploma
Submitted when requesting a replacement diploma.

Absence from Examination (available from the Registrar's Office)
Used to request an absence from an examination and to request permission for an alternate examination.

Change in Concentration, Department or Degree (available from the Registrar's Office)
Used when requesting a change in concentration, department or degree.

General Petition (available from the Registrar's Office)
Used to petition to change current student status. Doctoral students use general petition to register for research credit prior to the Oral Exam.

I.D. Replacement Form (available from the Registrar's Office)
Used to request a replacement of a Harvard I.D.

Incomplete Contract (available from the Registrar's Office)
Used by students receiving a grade of incomplete. This is a written contract between student and course instructor recording the negotiated work to be performed, the completion deadline, and the penalty, if any, to be assigned if the work is not completed.

Part-Time Credit Form (Credit Notification Form) (available from the Registrar's Office)
Used by part-time students to request any number of credits other than 5.

Prerequisite Wavier Form (available from the Registrar's Office)
Used when requesting to wave a course prerequisite.

Transcript Request Form (available from the Registrar's Office)
Used to request a HSPH transcript.

Waiver of Core Courses Form (available from the Registrar's Office)
Used to request waiver of BIO and EPI school-wide core course(s) only.
