Submit this form to request an IRB Authorization Agreement, which can designate HSPH as the Reviewing Institution (responsible for IRB review) or Relying Institution with any non-Harvard Catalyst institution.

Submit the following forms to request an IAA with MGH or BWH: Commitment Statement for Investigators, Site Responsible Investigator Form, and IRB Authorization Agreement Request Form. |

For IAA with Harvard Catalyst Insitution
|
Visit this link and use your eCommons or Harvard ID and pin, to request an IRB Authorization Agreement between HSPH and any other Harvard Catalyst affiliate. Submit this form to the IRB. |

Submit this form when researchers working on a project are not affiliated with HSPH, but are also not covered under another Federalwide Assurance-holding institution. These individuals will need to complete human subjects training. Also submit a Modification of Approved Human Research Form and an updated Study Personnel Information Form with your IIA request.
