New/Updated Forms (4/14/08)
Download new/updated versions of the following from our Forms and Tools Page. Contact us if you have any questions or experience any problems.
- Reapproval Application: Previous versions of the Reapproval Form will not be accepted after July 1, 2008.
- Appendix A: Study Staff Log/Staff Amendment Form - Use this form at the time of initial and reapproval to note the current study staff. This form should also be used for changes to study staff.
- Appendix B: Anticipated Serious Adverse Events Form - Use this form at the time of reapproval to note anticipated SAEs that have no relationship to research procedures.
- Human Subjects Research/Exemption/Clarification Request Form: Previous versions of this form will not be accepted after July 1, 2008.
Updated Submission deadlines (4/11/08)
- Renewal applications eligible for expedited review should be submitted approximately 45 days prior to the study's expiration date (please submit on the closest business day when that deadline falls on a weekend).
- Full Committee renewal applications, please refer to our Full Committee Meeting schedule and submission deadlines.
- All new applications, amendments, exemption requests, and responsive material will be reviewed on a rolling basis; submission deadlines are not applicable. Please refer to our submission turnaround times for anticipated review time.

IRB Clinic: HSC staff are available to answer IRB questions on Tuesdays from 1pm-3pm at the HSA office on Tremont Street (see Calendar page for details).

OFFICE HOURS FOR SUBMISSIONS: Mon-Fri (9:30am-4:30pm) at 1552 Tremont Street, Boston, MA. One original and two copies of all submissions are needed. We do not accept email submissions. Some exceptions may be made for special circumstances (e.g., researchers on sabbatical abroad and email is the only timely means of communication). Any paperwork submitted after the deadline will run the risk of lapsing.
New Resource for Investigators Working Transnationally: HSC has launched the Global Research Ethics Map, a unique web-based resource for HSPH researchers and the general public. The Global Research Ethics Map provides important information about conducting human subjects research in countries outside of the United States. CITI Training replacing RCR Class: For the academic year 2007-08, the Office for Academic Affairs is requiring individuals who need training in the responsible conduct of research (RCR) for their training grants or other purposes to take the free, online program "RCR" offered by CITI via the Office of Research Integrity website (www.ori.hhs.gov).
