Here is the REQUIRED sample submission form: WORD/PDF (form is 2 pages, please fill out both).

Help with filling out the form:

  • Contact information for you and your billing department are VERY important, please make sure these are filled out to the best of your ability.
  • Please provide distinguishable names for your samples and don't forget what is what!
  • Name the organismal/biological source of your sample (EX: if they are cells in culture, what organism is the cell line from?). Also include any relevant biological information which could affect the results or health of the technicians.
  • The more we know about your sample, the better chance of a good result. Our guidelines should help you create a usable and effective sample. Listing potential contaminants comes highly recommended.
  • If you do not know protein concentration and you are doing a comparative study, guessing is not wise. We can try to run an assay for you.
  • The selection chart is a way to specific conditions and materials for your project, HOWEVER, some elements will be special order because they are less common. The recommended choices are **-marked.
  • If for any reason you have problems or cannot access the form, let us know so we can help you or email a new form.
