IT Request Forms

  1. New User Account Request * – Use this form to request a new computer, email account, and shared resources such as folders, printers, mailboxes,etc.,  for an employee or contractor. The person will need to have an active Harvard ID. Note: all new requests are processed during business hours: Monday – Friday, 8:00 AM – 6:00 PM. Please allow 1-3 business days to complete. Email accounts cannot be created until the user has a MIDAS role.

  3. VDI Account Request – Use this form to become a user of the Virtual Desktop Infrastructure.
  4. Shared mailbox (Exchange/Outlook) – Use this form to request a shared Outlook mailbox. Outlook shared mailboxes enable multiple users to access, send, receive, and reply to messages, and as “on behalf of” the shared mailbox, as well as Calendars and “Rooms”.
  5. Network Drive Quota Increase Request – Use this form to request an increase the size of a department shared (S:) drive or a personal (P:) drive.
  6. Computer Setup Request – Use this form to request the setup and configuration of a new department-owned computer by a Helpdesk technician.
  7. Printer Setup Request – Use this form to request the setup and configuration of a new or replacement department printer by a Helpdesk technician.
  8. New Mailman/Listserv Request – Use this form to request a new email listserv to send messages to a list of subscribers. Mailman listservs do not have calendars, and cannot be used to schedule appointments.
  9. Emma Email Marketing Request – Use this form to express interest in Emma or request access to an existing sub-account.
  10. Computer Equipment Removal (PDF) – Print this form, fill it out, and bring it to the IT Helpdesk if you need to remove electronic equipment from campus.
  11. Computer Move/Transfer (PDF) – Print this form, fill it out, and bring it/ email it to the IT Helpdesk if you are planning on moving offices (either as a group or single person) and would like IT assistance.
  12. Static IP Request – Use this form to request a new static IP for a computer, printer, or other networked equipment.
  13. Media Services Request – Use this form to request future assistance from a MediaServices technician for support with a presentation, class, or event. For immediate AV assistance, please call IT at (617) 432-4357.
  14. Video Conferencing Request – Use this form to schedule assistance from a MediaServices technician for a video conference.
  15. Wasabi Access Request – Use this form to request Wasabi access.  For Wasabi password resets, please email Wasabi Support.
  16. Appointment – Book an appointment with a Helpdesk technician.
