Integration of care is a central challenge of health care delivery, particularly for older adults with multiple, complex chronic conditions. Not surprisingly, providing integrated patient care that is coordinated and patient-centered is emerging as a priority for healthcare system improvement efforts. Improvement toward this goal will require both research to identify effective strategies for promoting integration and mechanisms to ensure that organizations are accountable for their ability to provide integrated care. However, a paucity of valid and reliable measures of integrated patient care has hampered these efforts.

We developed a novel survey that measures integrated care as perceived by patients (the Patient Perception of Integrated Care or PPIC survey).
