St. Louis Baby Tooth Study – Surveys

Welcome to the Study Surveys Page for the St. Louis Baby Tooth- Later Life Health Study!

Thank you for your participation! Each survey you complete helps the study. Here is how you can get started:

For All Study Participants:

Haven’t completed the Enrolled Participant Information (EPI) Form? Please fill it out, regardless of your progress in other surveys.

• This is a brief survey that allow us to connect your data with all of your donated baby teeth and ensures we can reach you with future opportunities.
• If you ever need to update your contact information, provide feedback, or opt out of a part of the study, you can still use our Update Status Form.

If You Are Returning:

Already participated? If you’ve completed Survey 1 and its cognitive test (previously “Start the Study!” on our homepage), then it’s time for Survey 2:

• Like Survey 1, Survey 2 will start with a consent form and ask for your 6 digit study ID.
• When you click submit on the final page of the survey, you will be automatically redirected to your cognitive test 2.

If You Are New to the Study:

First time participant? Start with Survey 1:

• On the first page of the survey, you will see our consent form.
• Please scroll to the bottom of the page, enter your 6-digit study ID, and then click “Next” to start the survey.
• When you click submit on the final page of the survey, you will be automatically redirected to your cognitive test 1.
